SUMMARY

Aim - evaluation of the effectiveness and impact on the eye hydrodynamics of cataract phacoemulsification
with IOL implantation in patients with glaucoma.

Material and methods
Research was conducted on 81 patients (83 eyes) with complicated cataract and first open angle glaucoma. The
patients were divided into three groups.
The first group was 32 patients (32 eyes) with normotensive and compensated glaucoma. They were held
phacoemulsification with IOL implantation.
The second group included 25 patients (27 eyes) previously operated for open angle glaucoma. They also were
held phacoemulsification with IOL implantation.
The third group was 24 patients (24 eyes) which IOP was comphensated with antiglaucomatous surgery. They
were held extracapsular cataract extraction with IOL implantation.
The following factors were taken into account: degree of cataract and glaucoma development, IOP level,
hydrodynamic indicators, visual acuity, changes from the iris.

Results
Studies have shown that phacoemulsification of the cataract with IOL implantation in the eyes with previously
performed antiglaucomatous operations does not affect the IOP and hydrodynamics of the eye and allows to obtain
high functional results. With phacoemulsification, it is necessary to minimize the effect of ultrasound on the eye
tissue using a chopper, which reduces the load on the ligamentous apparatus and reduces the number of intra- and
postoperative complications and improves the quality of surgical treatment in these patients.
IOL implantation prevents intraoperative hypotension, and also due to constant positive pressure in the anterior
chamber, reduces the risk ofhyphema, coryochorodal detachment, expulsive hemorrhage.

Conclusion
Our studies have shown that phacoemulsification of cataracts in patients with glaucoma does not have
a significant effect on IOP and hydrodynamics of the eye, for a short period of rehabilitation allows for high
functional results.
