SUMMARY

Aim–to identify different cases of simulation and in each case, using appropriate tests or modern diagnostic methods to expose them.

Material and methods
The present study included 42 patients with various forms of simulation. The material was taken from The Military Medical Commission among young men aged 18-35 years (24 ± 4,5).To all patients there were conducted the following standard methods: visual acuity, refraction before and after cycloplegia, tonometry, slitlamp and indirect ophthalmoscope evaluation of an anterior and posterior segment of the eye. Depending on the nature of the simulation there were additional methods of examination: computer perimetry of visual feld (Tomey); electrophysiological studies (ROLAND CONSULT – Super colour-Ganzfeld 0450 SC, Germany) – electroretinography (ERG), the defnition of visually evoked potentials (VEP), optical coherence tomography (OCT (Cirrus HD OCT, Model 300)), corneal topography of the cornea, color test and encouraging the patients different tests, such as confusion or fogging were performed.

Results
Almost all ophthalmic sensory and motor functions including visual acuity, visual feld, color vision and night vision can be the subject of simulation and Munchausen syndrome to refractive surgery denial. Examiner must be skillful in selecting the most appropriate test. In each case to identify the simulation there were used the various tests and diagnostic methods, that is refected in this paper.

Conclusion
At present the simulation may be manifested with a wide range of spectrum from conversion to malingering. Currently, there are numerous forms of simulation. The examiner should be skilful at choosing the most appropriate test among those which are most preferred and easy methods used in ophthalmic practice. The use of modern diagnostic methods of examination reveal and objectively corroborates the simulation. However, the use of different tests such as confusion or fogging in the early stages of the examination makes it feasible to save the time and to use the expensive diagnostic methods.
